On My Watch Case Management

The goal of On My Watch Case Management is to connect provider and community resources to enable consumers to manage disease, remain in their communities and prevent premature admission to nursing facilities.

On My Watch has over 30 years of experience in delivering dedicated and innovative coordination services. Our proficient team works with eligible clients, caregivers, physicians, and approved providers to design client-focused care plans.

The Elderly Disabled Waiver Program assists Georgians who prefer to live in the community rather than enter a long-term care facility. Some people need assistance to remain independent in their own homes but do not need the 24-hour supervision and care provided in a nursing facility. That is why the Elderly Disabled Waiver Programs were developed – to give Georgians a greater choice in deciding where to live and what assistance they need.

A team assessment of your health needs and present condition will be made by a nurse and a physician. A care plan will be recommended to you and your family. If you choose to participate in the Elderly and Disabled Waiver Program, a care coordinator will arrange for services and evaluate your condition frequently. If your needs changes, appropriate changes in services will be made. If needed, we can assist with the Medicaid application process.

  • Have health and personal needs which can be adequately met in the community within established guidelines
  • Be an eligible Medicaid recipient. If a person has been previously determined ineligible for Medicaid, they may still qualify under the program because regulations differ. In some cases, clients may have to pay part of the cost of services. The amount is determined by the Department of Family and Children Services.

Personal Support Services

Assistance with:

  • Personal care and hygiene
  • Meal preparation and nutrition
  • Light housekeeping, home safety, shopping, correspondence, bill paying, and supplementary services
  • Securing transportation
  • Respite during absence of primary caregiver

Home Delivered Services

Provided are:

  • Skilled nursing services
  • Physical, speech, and occupational therapy
  • Medical social services
  • Home health aide assistance
  • Personal care and help with meals
  • Medically related transportation

Adult Day Health

Offers care and related services in an adult center for persons who need daytime care and supervision. Provided are:

  • Transport to and from center
  • Nursing and Medical social services
  • Planned therapeutic activities
  • Physical, speech, and occupational therapy
  • One balanced meal a day for full day participants

Alternative Living Services

Offer an alternative residence for persons unable to remain independent in their own homes. Provided are:

  • Board and personal care
  • Meals and supervision

Emergency Response System

Offers an in home electronic support system that provides two-way communication between isolated persons and a medical control center; available 24 hours a day, seven days a week.

Home Delivered Meals

Hot or frozen meals for homebound persons not able to prepare food for themselves:

Up to 2 meals per day, 7 days a week

How do I obtain case management services through On My Watch?

Is there a cost for services?

The Elderly and Disabled Waiver is a Medicaid-based program, but some consumers do have a cost-share. A care Coordinator will review your financial situation and advise you of any out-of-pocket cost during the initial screening process.

I don’t have Medicaid, what do I do?

Don’t worry! Our team of Care Coordinators can assist you with the Medicaid application process. Just give us a call and we will be glad to help.

How long does it take to begin receiving services?

Unfortunately there is no ‘one size fits all’ answer for that, but our referral specialists will work with local DFCS offices to make sure that you don’t have to wait any longer than necessary to receive services!

Why do I need to provide bank documents or other personal documents?

Since this is a service funded by Medicaid, there are income and asset limits for participants. This paperwork is collected by our team of specialists to make sure that you qualify for the waiver service.
